I understand that RHF may share program stories and participant experiences through various communication channels.

I consent to RHF using my photo(s), my video(s) and/or my quote(s) that I have submitted on RHF’s website, newsletters, social media channels, and in reports for program funders and/or supporters.

3. Consent to Share Information with CNIB

RHF partners with the Canadian National Institute for the Blind (CNIB) to support program administration, coordinated services, partnership activities, and reporting and evaluation.

I consent to RHF sharing my personal information, my photo(s) and/or my video(s) that I have submitted with CNIB for program coordination, administration, reporting, and evaluation. This may include use in CNIB social media, website, newsletters, donor communications, and similar public facing materials.

I may withdraw my consent at any time by contacting RHF and my withdrawal will apply to future use of my photos, videos, or quotes. Once materials have been published or shared, they may not be retractable from all platforms.
